Ideally, as a young teenager you began taking over some of the responsibility in the treatment of your cystic fibrosis. It is important to become familiar with the names of the medications you take and consult with your healthcare team about their possible common side effects so you are able to accurately report your symptoms to a doctor. You should also start to become accustomed early on with going to medical visits alone. Transition from pediatric to adult care generally occurs around the time of high school graduation.
Several strategies can be employed when making the transition from pediatric care to adult care. In almost all cases a gradual transition has been most successful, often marked by a period of time spent in both pediactric and adult facilities. Abrupt transitions have been seen to compromise the effectiveness and commitment to treatment in the adult care setting. In any case, cooperation and planning amongst the pediatric care team, the adult care team and the patient and the patient's family is essential for a successful transition.
